Unfortunately due to contractual requirements and auditing, we are unable to take request for repeat prescriptions via telephone as we have to have a paper record of all requests 

To obtain a repeat prescription please keep the slip on the right hand of your prescription and tick the relevant boxes for the items you require. You can then either drop it in by hand to the reception or post it to us.
 
we have a quick and simple form that can be filled in at reception.

Please allow 48/72 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
